I was proactively involved in a strategic quality improvement project aimed at improving patient discharge education, decreasing 30-day readmission, and the interdisciplinary collaboration within an inpatient hospital context throughout the 50 practice hours. To evaluate the current structure, safety practices, and leadership model of the organization, I started the SOAR (Strengths, Opportunities, Aspirations, Results) analysis. This involved taking part in interdisciplinary safety huddles, checking discharge protocols, considering patient satisfaction, and evaluating patient satisfaction data to discover areas of improvement.
Most of my time was devoted to working with nurse managers, case managers, pharmacists, and IT staff to comprehend the workflow on discharge planning and Electronic Health Record (EHR) tools integration. I witnessed the method of teach-back education at discharge and observed the obstacles, like time restrictions or communication issues, and assisted in creating the strategies to enhance staff participation. I also engaged in stakeholder meetings during which I delivered data-based findings and participated in the planning sessions intended to generate culturally competent discharge resources and onboarding guidelines regarding Transitional Care Nurses (TCNs).
I also collaborated on designing cultural humility, teach-back communication, and documentation-training modules to be used by staff. I analyzed patient education materials and helped in converting them into the five leading patient languages used in the facility. Simulation activity, analysis of the feedback of orientation and performance reviews on quality metrics in terms of performance dashboards, were also part of these practice hours. The 50 hours were an extensive, practical experience to apply leadership, communication, and systems-thinking skills in a clinical environment that aims at the enhancement of patient outcomes and safety.
