With good financial management, it is also possible to give good healthcare services and meet the objectives of the organization in any healthcare organization. Natali et al. (2025) determine two simple types of budgets in health care. The budget systems can be classified into two: investment project budget (capital budget) and the daily section (operating budget). Budgets and budget control are crucial elements of effective financial sustainability, efficiency, and quality.

    Ongoing Management of Capital Budget

    Capital budgeting management is a constant process that involves monitoring and measuring costs, implementation processes, equipment use, maintenance expenses, and payback performance. The capital leadership of nursing organizations keeps an eye on the deviations and the performance of the capital to guarantee effectiveness in capital use (American Nurses Association, 2023). Evaluation of capital costs, equipment utilization, and cost to maintain optimal performance helps healthcare organizations to analyze the performance and return on investment.

    The capital expenditure of $ 16.6 million is paramount in the process of fulfilling the objectives of the organizations pertaining to efficiency, safety, compliance, and sustainability. The performance budget containing equipment usage, maintenance cost, and patient flow, among others, is examined to decide whether there have been any gains made out of the capital expenditure. There are always some troubles that result in an increase or reduction in the budget, time, and allocation of resources (Hamed, 2023). The current inquiry into capital expenditure can be used to enhance the performance and even sustainability.

    The objective of the operating budget is to control the expenses of the health care organization is achieved and that the patient should be satisfied with the operation of the health care organization. Cost elements in the budget prepared to undertake operations, which focus on salaries and benefits of the employees, are the major ones, due to the objective of making sure that there is an adequate number of nurses. Moreover, organizational objectives such as stability, efficiency, and safety have been met by using operating budgets. Using the analysis above, the operating budget, the projected operating income will increase to approximately $52 million in the upcoming fiscal year, as opposed to the current fiscal year, projected to be about $50 million.

    The operating costs will begin to inflate to about 49,717,500 due to the increase in the cost of staffing, remuneration of personnel, materials required to accommodate the requirements of healthcare delivery, as well as an augmented cost of living (see Table 2). But even with more overhead costs, the operating income will be about 2,282,500 in this business. The reasons for adopting the use of the above operating budgets are nurse planning, financial planning, control of the true cost of the health care budget, and efficient healthcare service throughout the year (Heydari et al., 2022).

    Areas of Uncertainty

    Other potential sources of financial risks may be the disparity between the demographic pattern of patients, variations in the compensation rate, higher costs because of inflation, employee shortages, and healthcare industry regulations. As an example, any alteration in the level of payment provided under Medicare and Medicaid programs will have an enormous effect on the budget of the organization. The healthcare organization should consider financial forecasting, variance analysis, and contingency plans to handle financial risks (Fitriasari et al., 2025).

    Operating Budget Design and Creation

    An operating budget is prepared based on the financial performance of the previous years, the actual expenditure on an annual basis of the current year, the number of patients to expect the following year, and the level of inflation anticipated in the operations of healthcare. There will be an increment in salary amounting to over 1, 300,000 due to the fact that the organization has tried to maintain that there is adequate staffing that will help in quality health care (see Table 2). Additionally, the expenditure incurred in ensuring employees’ benefits will increase by $6,720,000 in a bid to ensure that employees stay in their jobs.

Nurse managers liaised with the accounting department to make sure that operational expenses were in line with the organization’s goals.

    The other conflict areas in the organization were the financial and operational conflicts, which resulted in the formulation of the operating budget of the organization. While there were projections for the growth of the operating income of the company between $50,000,000 and $52,000,000, there were also predictions of much greater growth in the organization’s expenditures because of the growing costs of salaries and related expenses, supplies, and increasing health care costs in the industry.

The reason is that they must consider finances at the same time, catering to the increasing needs of safe staffing and efficiency. Some challenges were experienced in the process of meeting the target of maintaining expenditures low without reducing the staff members and jeopardizing their safety.

    Process for Calculating Cost

    Estimates on cost were determined using historical trends, estimates of patient volumes, inflation, and the productivity of the labor force. It was forecasted using past cost as used in the past, the productivity of the workers, the number of patients who are estimated to be served, and inflation. According to variance analysis, it was established that the increase in cost projection was a result of the labor and supplies needed. The cost of operation in the future was determined using a forecasting technique. The staff, pay per hour, overtime, and percentages of employee benefits were used to predict the salary costs.

    As an example, the salaries and wages expenses that were estimated to cost $26,000,000 at present had escalated to 27,300,000 considering the increased number of staff and patients having a high acuity rate. The difference between the actual and estimated cost figures was calculated by using variance analysis. The nursing administration studies have stated that variance analysis and financial forecasting are beneficial in enhancing operating efficiency without making compromises on patient care (Ismail et al., 2025). The results of the variance analysis of the current year revealed that the estimated cost of operation was approximately 20,930,000 lower than the operating budget of the current year, halfway through.

    Ongoing Management of Operating Budget

    The operations budget will involve conducting a regular review of the cost of labor, departmental efficiency, supplies, and cost differences. A review of the variance report showing the difference between the expected costs of the company and the actual ones is done monthly. Periodic evaluations of the cost of labour, overtime, and supplies are important since they help confirm that actual costs are matched with the anticipated costs of budgeting.

    This will ensure easy decision-making and will make it easier to sustain the company by nurse administrators. The methods that will be used include preventing overtime expenses, supply partnering, and staff planning without interfering with the health results of the patients. Budget evaluation is going to make it easier to achieve the goal of the organization on patient safety, proper finance, regulation, and staff retention (Homauni et al., 2023).

    Role of Nurse Leader and Considerations of the Organization’s Financial Sustainability

    Nurse leaders’ operational budgetary role assumes a fundamental role in the assessment of the distribution of the budget that serves as the cost of labor, overtime payments, patient acuity, and department productivity. They are required to evaluate matters that concern the productivity of the staff, payment of overtime compared to patients’ acuity, and, as nurse leaders, to facilitate staffing based on the needs of the patients and organizational goals (Ismail et al., 2025).

    Effective budgeting helps nurse leaders to allocate their resources easily and deliver quality healthcare services. The largest amount spent in the budgeting process is labor and staff costs; hence, a sufficient labor force should be capable of achieving organizational goals. Nurse leaders liaise with the finance department in reviewing disparities in costs and take the necessary action in case of surplus in costs and deductions compared to the expected amounts on a monthly basis.

The operational budget is crucial in operations regarding the services offered to the patients in terms of health care. Capital budget, on the other hand, is a very important aspect that can assist healthcare organizations in expanding and growing by investing in projects that will lead to growth. Involved in budgeting are constant financial management, variance analysis, forecasting, and communication between nurse executives and financial managers. Budgeting helps organizations to meet their goals, such as providing safe and quality health care services. With financial management and evidence-based budgeting, organizations will be successful in meeting their objectives.
