By being informed and taking initiative in financial management, nurse managers can contribute to the success of an organization by budgeting effectively.

    Strategic Planning and Its Impact on Budget Creation

    Strategic planning will offer the blueprint of the way health care organizations should develop their budgets and administer their budgets. It can also assist the nurse leaders in recognizing the organizational priorities and converting them into quantifiable financial goals. Margiutomo et al. (2025) state that the strategic planning step could match resources to strategic objectives, which are chosen on the basis of the vision and activities of an organization. An organization lacking a clear strategic plan is not able to come up with an active budget and instead becomes unproductive.

Nurse leaders keep track of variances and frequently make adjustments to resources based on the variances and conduct cost-benefit analyses on a daily basis. Making the right and evidence-based budget decisions involves comparing actual expenses to future expenses (Melnyk et al., 2023), which is why it is vital for nurse leaders to do this. When nurse leaders take proactive steps with the budget, they will have the opportunity to deliver quality patient care without financial constraints.

    As a nurse leader, you need to be aware of all sources of funding to your health care organization, as well as all types of payors. A number of revenue sources (i.e., Medicare, Medicaid, private insurance, and self-pay patients) will be available in your health care organization. Any alteration or variation in your organization’s payor mix may have significant impacts on your revenue in the organization and the organization’s operational capacity. Pradhan et al. (2024) state that this payor mix could determine the revenues and financial performance of an organization in general. The payor mix data should be reviewed and utilized in a proactive way by nurse leaders since at that time, nurse leaders can predict any significant changes in revenue, which will enable the organization to continue being profitable in the long-run perspective.

    Components of an Operating Budget

    A comprehensive operating budget is a financial statement, the details of which, according to Kohzad (2024), is the most vital, and aids in the daily consumption and resource management of decisions of organizations. If a complete breakdown of each expense is provided, nurse leaders can control and be efficient in managing the operation and the finances.

    Components of a Capital Budget: Fixed Assets and Equipment Costs

    Operational costs of a daily need refer to costs that are not personnel costs to provide safe and quality care. Daily operating costs include the medical supplies, pharmaceuticals, repairing equipment, and other administrative costs that are constantly incurred during a fiscal year in offering health care. The organization at large will be in better financial health and sustainable through setting controls of hourly and daily operating costs.

(2023) highlighted that Organizations that have not included the long-term cost of the service contracts in their Capital Asset analysis will experience huge budget overages and high budget compression in the organization. Thus, Total Cost of Ownership (TCO) must be included in the process of evaluating and granting capital expenditures by Nurse Leaders.

    Comparing Operating and Capital Budget Management

    Budgeted capital and the operating budget complement one another and cannot run without the other since they supply financial resources to the organization. The routine costs of the operation of the organization are contained in the operating budget and a strategic investment that has to be planned and approved is contained in the capital budget. Lee (2023) states that nurse leaders need to allocate both their labour and service resources accurately and strategically to manage both budgets effectively. By operating under a single operating budget and single capital budget, the nurse leaders will be able to make their organization even stronger financially to carry on providing high-quality and sustainable healthcare services to patient populations.

    Budget management and capital costs are the two types of financial constraints of nurse leaders. As an example, assuming that the nurse leader wants to use the operating budget to make labor resources available enough to ensure that the patients are properly staffed, they will also be forced to spend capital funds on large capital purchases (e.g., equipment); hence, it would be hard to balance the operating and capital side of the financial resources in the mind of a nurse leader.

    When well-planned and executed, capital purchasing can have a large effect on an organization’s financial position and future operational capabilities.
