At the Cleveland Clinic, I completed a 50-hour practicum where I helped with multiple nursing informatics projects to support the advancement of health care quality and safety and the improvement of clinical workflows. This offered the opportunity to be part of the nursing informatics team with a focus on the use of technology with patients and the collection of various data to assist the different disciplines in providing evidence-based solutions.
A large part of my practicum involved understanding the Electronic Health Record (EHR) and how it’s utilized in clinical documentation and the administration of medication and care plans, as well as in interprofessional communication and documentation. I learned how to review documentation and identify errors and gaps in documentation and how to develop documentation that aligns with the standards of the organization. I also learned how Clinical Decision Support Systems (CDSS) prompt/deliver time-saving alerts, reminders, and recommendations to help clinicians avoid making errors and improve the experiences of the patients provided by the system. From my practicum, I learned how to conduct multidisciplinary meetings to provide additional input for several technology initiatives that are in the planning and implementation stages through the combined lenses of nursing, informatics, quality improvement, and IT.
The focus of these meetings was on the optimization of the EHR and the improvement of user satisfaction of the users, workflow, and quality indicators. From these experiences, I learned how to effectively plan and implement a large-scale project, the importance of engaging the key stakeholders, the necessary system and change management, and the ability to adapt to the complexities of a large healthcare organization.
Another area of my work where I was fortunate enough to receive a review was the reporting of important clinical metrics such as Patient Falls, Hospital-Acquired Infections, Medication Safety, Documentation Compliance, and Patient Satisfaction. Guided by my preceptor, I reviewed the quality data trends and discussed these data in relation to the current quality data improvement initiative and how the informatics (IT) tools would assist. I learned how to capture and maintain data in a standardized format, which enables the organization to deliver services that are legally compliant and patient-centered and to fulfill the reporting requirements of the organization.
As part of my practicum, I helped make additions to the EHR and the education module. I also helped design teaching materials to encourage desirable practices of documentation in and use of clinical information systems. The experiences also helped outline the framework of adult education for me, and differentiated the concept of education/trainings to the clinicians and the adoption of the technology.
Ultimately, the practicum provided me with a valuable perspective of the nursing informatics competencies and the health care information systems, data analysis, and quality improvement. The practicum closed with implementable strategies to enhance adoption and workflow fit, and offered approaches that assist communication between patients and their healthcare teams.
