    Hello, my name is _______ and I would like to present my person-centered self-care management interview that is going to be devoted to the sphere of motivational interviewing and the ways it could be applied to motivate patients to be more empowered and healthier.

    Slide 2:

    Motivational interviewing is an approach of respecting and aiding communication aimed at assisting patients in identifying their emotions and taking action to improve their well-being. It pays attention to listening, empathy, and helping the patient to have realistic objectives (Bischof et al., 2021). By positioning myself as an advanced practice nurse, I will aim at establishing a trust relationship, which results in confidence and self-care. The presentation will entail the chronological description of my experience of applying motivational interviewing to one patient who had to be assisted in self-care management.

    The type of patients I also met during my practicum included a person with type 2 diabetes, who did not track the daily sugar levels and diet changes. The patient was feeling frustrated and fed up that her efforts were not bearing fruit, and she did not have a sense of motivation. I also adopted motivational interviewing in order to investigate the issues that were being faced by the patient and the need to change.

    I did not make any direct instructions but opened up with questions so that the patient could open his/her struggles and ideas. Such an approach contributed to making the patient feel respected and supported. The teamwork process also helped us to define the issues, talk about the possible options for solving them, and create self-care achievable goals that included the use of the technology to make the process easier.

    Strategies to Promote and Foster Caring Relationships

    A nurse and a patient relationship is based on the caring relationship, which presupposes that there will be trust, empathy and respectful conversations. Active listening will enable the patient to feel special and heard, which will contribute to more cooperation and engagement in treatment (Tennant et al., 2023). Open-ended questions will assist the patient to express their experiences and problems, and this will further assist in understanding what hinders self-care. Affirmations focus on the positive aspects of the patient and the past that help the patient to be more confident and eager to do something new (Bischof et al., 2021).

    Reflective listening is empathetic as it echoes or summarises issues; this will assure the patient that his or her feelings are good and worthwhile. I have used the techniques on my patient, and it is assured that motivational interviewing would lead to improvement in engagement, encouraging autonomy, and reducing resistance to change towards better health outcomes (Bischof et al., 2021). The caring relationships must also be facilitated by the creation of a safe environment that would enable the sensitive issues to be discussed without stigmatisation. These plans helped me to empower my patient and make her believe that she can cope with her health responsibilities.

    Strategies for Facilitating Difficult Conversations and Disclosure

    In order to discuss a challenging matter, it is necessary to know how to communicate empathetically, trustfully, and confidently to establish a discussion. Open-ended questions enable patients to discuss their issues and are not coerced, which eventually results in the identification of self-care barriers (Tal and Epstein, 2021). Reflective listening makes the patient feel that his/her feelings are acknowledged and honoured, which causes a lack of defensiveness and doubt.

    I applied such a method to my patient when he/she stated that he/she was frustrated because of glucose monitoring, and it resulted in a constructive conversation as opposed to resistance. Caring and respectful communication has been demonstrated to enhance the sharing of sensitive information and the exchange of therapeutic relationships. A secure environment should also be developed when discussing sensitive issues. The truth will be elicited using the secrets of confidentiality, noble language, and positive nonverbal communication, such as eye contact. Empathic communication enhances patient satisfaction, decreases stress, and equips them to divulge personal information (Thompson et al., 2022).

    It is better to have difficult conversations, where the nurses should aim to listen to the patient and provide no response at this stage. With such strategies, the nurses would be in a position to make sure that such hard conversations enhance compassionate care and result in better self-care outcomes.

    Strategies that Promote Self-Care Management

    The SMART goals developed to be provided to the patient with type 2 diabetes were focused on improving consistent self-care in real-life. The patient was recommended to maintain the level of sugar in blood daily and measure it using a glucometer and a proactive attitude toward disease management. It has been made easier through a mobile health application that would automatically transform readings to minimize the tendency of using manual forms and the application would also present a clear picture of progress. Integrated reminders helped to maintain the communication and interaction with the care routine. In an effort to enhance understanding, the plain language, audio narration and application of repetition of vital information was employed to simplify learning material and reinforce learning to support comprehension (Smith et al., 2023).

    Equity and inclusiveness issues were also included in the plan, particularly the personalization of dietary advice to align with the cultural food preferences of the patient as this would contribute to more acceptability and sustainability. This system was an effective memory system and the patient could easily monitor her progress as time went by. It has been reported that digital health tools could support adherence and confidence to handle chronic condition management (O’Hara et al., 2022), and the technology in this instance assists in normalized routine monitoring. It also minimized the major hurdles associated with it because it provided graphic representations and streamlined information, and even people with low health literacy could understand it.

    Inclusion was further promoted by the multilingual features, which guaranteed usability by the various backgrounds. This strategy combined trusted online resources and an accessible layout to close knowledge and interaction gaps and finally enhance better clinical health outcomes in the long-term.

    Applying Counselling Techniques to Advance Wellness

    This method employs open-ended questions, affirmations, reflective listening, and summarising in order to establish trust and confidence. As an example, a patient with diabetes can analyse the obstacles to monitoring blood sugar and come up with a reasonable approach to changing the situation with the help of the nurse. As it has already been determined, motivational interviewing enhances self-care and health outcomes among chronically ill patients.

    The advantages of counselling methods are not limited to the short-term behavioural changes. Some of the most important materials to monitor the progress include the affordable medications, education tools, glucose monitors, support groups, and mobile health tools. Having these resources available on the board, the patient will be able to track their conditions, adhere to their treatment plans, and make healthier everyday decisions. This is a burden on access, health risks, and adherence to self-care due to the inability to access these resources (Baryakova et al., 2023). Nurses are involved in gap identification and patient referral to the relevant support.

    In other cases, the resources can be constrained due to a lack of financial resources, inadequate transportation or technology. The solutions can involve the involvement of patients in health programs in the community, the distribution of paper-based tracking devices, or group education. The use of telehealth services can also be facilitated by nurses and will be targeted at the rural or underserved populations. Approximating the available resources and bridging the gaps, nurses can draw a conclusion that the patients have equal opportunities to manage their self-care.

The strategies reduce the barriers, build confidence and provide different patients with a fair opportunity to enjoy a happy life. Evidence-based care and compassion play an essential role in helping with self-care management and positive health outcomes in the long-term.
