    Abstract

    Hospital-acquired pressure injuries (HAPIs) have always been a problem in acute care and are often linked to shortcomings in the guidelines for prevention and care. When a 30-bed medical-surgery unit noticed a significant rise in the number of HAPIs, it implemented a quality improvement project to determine if real-time electronic health record (EHR) analytics using the Data-Information-Knowledge-Wisdom (DIKW) model would help as a tool for nursing decision-making and reduce patient harm.

    The live compliance dashboard, cell phone workflow alerts, and automated risk warnings were also part of the intervention, with the intent of improving prevention behaviors. Braden Scale score result, skin assessment result, and adherence to re-positioning were actively provided as feedback to nurses, with hourly trend data provided to ascertain high-risk times, such as less repositioning at night shifts.

    To address these gaps, there were staffings and shifts, with the specific mandate of a daily safety huddle to ensure this adherence was being met. Standardized nursing terminology, such as NANDA-I, was used to document the risk of impaired skin integrity and to guide personalized care planning and intervention that was patient-centered. There were measurable outcomes of the initiative, including a 42 percent reduction in the incidence of HAPI after 90 days and a 35 percent improvement in compliance with the preventive bundle.

    These results endorse the fact that the implementation of structured models of informatics and standard terminology can convert raw data into knowledge that can be used to take immediate action in response to patient risks. In conclusion, the project illustrates the use of the DIKW model and NANDA-I for proactive nursing, improving patient safety, and facilitating change in care quality in a sustainable manner.
