I have completed 50 practicum hours at the Cleveland Clinic, working in the outpatient environment of the internal medicine department and behavioral health collaborative care setting. This practicum site was an experience as it provided me with an opportunity to work with a diverse adult community with acute and chronic medical and mental health problems. All the hours would be worked under the supervision of an approved preceptor, and would not be considered as part of any employment requirements.

My practicum experience involved an extensive examination of a patient and gathering of his whole history, physical examination, mental health, and medication history. I also assisted in the management of chronic illnesses, which included hypertension, type 2 diabetes, hyperlipidemia, anxiety disorders, and major depressive disorder. I had the opportunity to develop differential diagnoses and support with evidence-based treatment plans to accommodate the current approaches to healthcare and individual specifics.

The greater part of my practicum experience was connected with the incorporation of behavioral health in primary care. I also participated in the depression and anxiety screenings with the assistance of standardized tools, assessed the medication adherence, and provided counseling to patients on how to cope with stress, change their lifestyle, and learn coping skills. This experience helped me strengthen my proficiencies to treat physical as well as psychological aspects of health and facilitate holistic care.

I have also been working in an interdisciplinary team that comprised physicians, nurse practitioners, social workers, and pharmacists. The fact that I had the experience of working in a team based on patterns of a case discussion enhanced my understanding of coordinated care planning, especially the work with patients having complex medical and psychosocial needs. I was able to practice in assessing the outcomes of laboratory tests, diagnostic tests, and treatment outcomes to determine the progress of the patient.

I seized my opportunities during the practicum to discuss with my preceptor clinical reasoning, assessment methods, and clinical communication skills to enhance my skills. This state-of-the-art healthcare allowed me to enhance my competence in carrying out comprehensive assessments, chronic illnesses management, behavioral health integration, and teamwork, which allowed me to widen my duties in advanced nursing practice.
